At Positive Life Choices, we provide compassionate social work and counselling support to older people receiving Support at Home funding, helping them navigate change, strengthen wellbeing and remain connected to the people and communities that matter to them.

Support at Home

Positive Life Choices provides social work and counselling services for older people receiving support through the Support at Home program, where the service is appropriate and included within their available funding and support arrangements.

We can work with individuals, families and aged care providers to explore whether our service is appropriate and how it may complement existing supports.

Working with Aged Care Providers & Professionals

Positive Life Choices can work alongside aged care providers, GPs and other professionals to provide additional psychosocial and social work support to older people and their families.

We can assist when a client is experiencing:

·         Family conflict or communication difficulties

·         Grief and bereavement

·         Social isolation

·         Carer stress

·         Adjustment to changing circumstances

·         Increasing support needs

·         Difficulty making or communicating decisions

·         Reduced confidence or independence

Referrals are welcome from aged care providers, GPs, health professionals, families and care partners.
